Consent to Release Records to Third Parties (FERPA) – web form- click link; used to provide consent which allows third parties access to a student’s education records

Math Repeat Limit - University level petition used to request approval to register for either MATH 1111, MATH 1113 or 2211 after maximizing the two attempt policy.

Overload Request Form – used to request approval to register for more than 18 credit hours in one semester. This form is for seniors (90+ hours) who have a 2.0 cumulative GPA or higher.

Cross-Level Registration Request– used by a degree-seeking or post-bacc Atlanta campus student who wishes to take classes at a Perimeter campus during the summer semester

Transient Student Request Form – used by a degree-seeking student to request application to another institution as a non-degree seeking student in order to take classes for a specified period of time as a visiting student

Petition to Graduate in the Current Term – used by students who have applied for a future graduation term to request approval to graduate in the current term. Receive form from your academic advisor once eligibility is met.

University Level Petitions– used by students who are requesting policy waivers and variances on the University-level undergraduate academic regulations. Receive form from your academic advisor once you have discussed your petition request.
