Graduate students in the College of Arts & Sciences are responsible for monitoring their academic evaluation for accuracy, including the completion of coursework, status selection forms, non-course milestone forms, and final project forms.
Please use the forms below to notify us of a change to your Master's thesis/non-thesis status or program concentration, and to indicate successful completion of the appropriate non-course milestone and final project. If you have questions about any of these forms, please contact your advisor in the Office of Graduate Services. If you have questions about your coursework, please contact your academic program advisor in your home department.
Status Selection Forms
Use the form below to let us know whether you are a thesis or non-thesis Master's student. This should be done in consultation with your graduate program director. Please note that not all departments have both options. Once completed, the form will be submitted to the Office of Graduate Services and your graduate program director will be contacted for approval. You will be notified once the request has been approved.
Use the form below to formally change your concentration track. This should be done in consultation with your graduate program director. Once completed, the form will be submitted to the Office of Graduate Services and your graduate program director will be contacted for approval. Required Non-Course Milestone Forms
Please use this form to report successful completion of your language or research requirement. Not all programs have such a requirement, so please consult your departmental handbook for specifics. Forms should be submitted to the Office of Graduates Services immediately upon completion. Final Project Forms
Use the web form below to report the completion and final approval of a master’s non-thesis capstone paper or project. This form can not be submitted until the project has been successfully defended (if applicable), completed and your committee has approved. Upon submission, your form will be automatically routed via email to your primary advisor for confirmation that your final project has been approved. The email response we receive from your primary advisor will serve as his or her signature for the project. Please note, we must receive email confirmation from your primary advisor by the appropriate degree requirement deadline.
Given the suspension of face-to-face instruction resulting from the Covid-19 pandemic, the university has issued guidance that all thesis and dissertation defense activities should be held remotely. In this environment, we acknowledge that physical signatures on the final thesis/dissertation approval form will be difficult to obtain. Consequently, we are offering two alternatives for ease of submission.
Option 1: Electronic Signatures (preferred option)
Use the fillable PDF form link below to report the completion and final approval of a doctoral dissertation or master’s thesis. Electronic signature will be considered valid. You may forward the same form to each member of your committee as well as your department chair in succession for his/her electronic signature on the same form, or you may simply collect one signature per form: in this instance, make sure you combine the forms into a single multi-page PDF.
Doctoral Dissertation or Master's Thesis Approval Form (PDF)
Once you have obtained electronic signatures from all parties, the PDF must be submitted electronically to the Office of Graduate Services by the appropriate degree requirement deadline, using the drop box below. If you have questions about your final project form, please contact your advisor in the Office of Graduate Services.
Option 2: Email Approval
Only use this option if your committee members do not have the ability to produce electronic signatures. Each committee member must email their vote to the chair of the committee. The chair will then aggregate and email these responses to the correct advisor in the Office of Graduate Services. Please note, we must receive email confirmation from your committee chair by the appropriate degree requirement deadline.
Graduation Exit Surveys
The College requires that all graduating master's and doctoral students complete a brief exit survey. This survey will ask questions regarding your experience as a graduate student at Georgia State as well as your future plans. It is important to take the survey at the end of the semester, following the submission of your final project.
